Requirements for a Licensed Practical Nurse (LPN):
Valid PA LPN License or Valid PA RN License Physical within one year 2 Step PPD or Chest X-Ray Valid CPR Certification Valid Driver's License Responsibilities of a Licensed Practical Nurse (LPN):
